Biggest Loser is NOW CASTING TEENS 14-17 years old with 75lbs or more to LOSE!
THE DEADLINE FOR SUBMISSIONS IS OCTOBER 3RD, 2011

1.) Contact Information – Name/Age/Height/Weight/City&State/Phone&Email
2.) Bio Questions
-What struggles/obstacles do you deal with on a daily basis? What has the weight held you back from?
-What is your motivation for losing the weight? What are you looking forward to once you lose the weight? Explain in detail.
-What do you like to do for fun? Are you involved in any activities (sports, clubs, organizations) at school? If so, explain.
3.) Hometape Footage:
Please submit a 2-3 minute hometape via youtube.com or vimeo.com giving us a brief idea of what you are all about and why you need this opportunity. This is a chance to show us your personality so be sure and have fun with it!!
Be sure to include the following in your hometape:
-Name, age, height and weight
-What grade you are in and what you are involved in at school (if anything)
-Why you need this opportunity and what makes you deserving of it
-What you like to do for fun (show us if possible)
-Anything else that you feel is relevant to your story that you wish to include/show
General hometape notes to remember:
-Have someone else film you. We want you in the frame at all times. Do not sit in front of your computer camera the whole time. Get up and move!
-Make sure you are well lit. We need to be able to see your face. Show your personality
